Strategic Care Solutions (SCS) is a top senior living management company in New England organized to combine the core services needed for comprehensive management of Skilled Nursing Facilities and Senior Housing Services. Some clients engage SCS to provide long-term management services; others engage us for short-term or interim contracts for episodic needs. David Roush
David Roush has worked in health care as an attorney, regulator, consultant, operator-owner, investor, and public policy advisor.
Mr. Roush co-founded Strategic Care Solutions (SCS) in 2013 and has served as CEO. SCS is a New England based consulting firm- which assists health care and senior living clients manage strategic, operational, business, project, and compliance challenges. SCS’ clients include Hospitals, Hospital networks, and providers of post- acute, skilled nursing, specialty SNF, memory care, assisted living, CCRC, and senior living services. SCS has advised government, banking- financing, and advocacy groups. Mr. Roush has guided SCS in building a Team of diverse subject matter experts associated with administration, marketing, clinical care, business office and financial services, compliance, strategic planning, grant application, and service innovation.
Mr. Roush ‘s consulting work focuses on strategic planning, project planning, service innovation and regulatory compliance. He oversees SCS ‘regulatory and compliance facility work outs. Since 2022, Mr. Roush’s particular interest has been in seeking the development of new models of care to address the emerging access crisis between hospitals and SNFs, and in advocating for the modernization and redevelopment of post-acute facilities, such as small house SNFs.
Prior to 2013, and starting in 2000, David was the co- founder and President -of Radius Health Care, a privately held, Massachusetts based elder care ownership and management company. At the time of its sale in 2013, Radius owned and managed 16 facilities in Massachusetts and Rhode Island and employed more than 2,000 professionals and caregivers who worked in Radius ‘SNF, hospital based post-acute, LTCH, assisted living, and memory care programs. Radius facilities had a documented record of high-quality survey results for more than a decade. Eight Radius SNFs executed facility redevelopment renovation projects.
Before forming Radius, Mr. Roush’s work focused on leading two independent health care consulting practices: ADS Consulting -and Roush & Associates. Both consulting practices featured niche services for Hospital, SNF, senior housing, CCRC, assisted living, and government clients. Consulting work included the development of more than 40 Hospital based post-acute service programs, the receipt or more than 30 Determination of Need /CON project approvals, the start up of a dozen “replacement” facility SNFs. Compliance work included the successful management of major quality of care compliance turnarounds, including CMS and State compliance monitoring, receiverships, and temporary management assignments. Roush & Associates assisted four premier RI not for profit elder care groups- found an innovative management services organization – Care Link. As a consultant, Mr. Roush co – authored two professional advisory manuals for Thompson Publishing Group: “Managing and Developing Senior Care “related to development of assisted living; and “Managing and Developing Physician Networks, for hospital, physician group clients. Mr. Roush started his professional career working for the Massachusetts Department of Health. During this span, he helped Massachusetts regulators develop a new Patient Abuse Reporting System for SNFs, and institute a new DPH “suitability” licensure process for health care ownership transfers. He served as Massachusetts’ State Agency Coordinator for the SNF Patient Care Receivership program. Mr. Roush was recognized by CMS for his special contribution in helping craft and design the framework of the Federal SNF Enforcement system in 1992.
Mr. Roush is a graduate of Columbia College and Boston University School of Law. He has been a member of the not-for-profit board of Vineyard Village at Home- which serves seniors at home with transportation needs. In 2024, he has been appointed as Chairman of the Board – of Navigator Homes of Martha’s Vineyard, a not for profit, which is sponsoring the development of an innovative small house- Green House SNF to replace an existing Vineyard SNF program in 2026.
Dolores Schermer
Dolores Schermer joined Strategic Care Solutions as a Partner in 2014 after 15 years with Radius Executive Services and Radius Management Services, where she served as Senior Vice President of Operations and Chief Operating Officer. Radius Management Services owned and managed over 2000 beds in New England and Rhode Island and was recognized as a quality company providing both staff and residents with a healthcare delivery system fostering individual health and wellness. Ms. Schermer has more than 20 years’ experience in the field of elder care management.
Ms. Schermer founded Sophisticated Seniors.com, an on-line company which promoted home environment safety and marketed adaptive products for seniors. She has served on the Board of Directors of the Alzheimer’s Services of Cape Cod & the Islands, and on the Board of Directors for the New England Senior Care Association.
Ms. Schermer holds a BS in Anthropology from Hunter College and an MS in Educational Administration and Supervision from Manhattan College in New York. She attended the Continuing Care Administration Professional Studies Program at Northeastern University.
Richard Blinn
Richard Blinn joined Strategic Care Solutions as a partner in 2020 after 21 years with Genesis Healthcare Corporation, where he served as Executive Vice President/President of the Northeast Division. Genesis Healthcare is one of the nation’s largest post-acute care providers with nearly 400 skilled nursing centers and senior living communities in 26 states nationwide.
As Executive Vice President/President of the Northeast Division, Mr. Blinn oversaw daily operations of over 120 Genesis HealthCare Centers and service sites, community-based programs, and physician services throughout New England. Mr. Blinn also oversaw all business development and expansion of the Genesis HealthCare Network in the Northeast Division. Mr. Blinn has over 40-years of experience in the management and operations of eldercare services.
Mr. Blinn holds a Bachelor of Science from Westfield State College in Westfield New England and a Master’s in Human Service Administration from The University of Southern New Hampshire in Manchester, New Hampshire.
Mr. Blinn is a New England Senior Care Association, Past-Chairman; Past President of the New England Long Term Care Foundation;past Board Member of the New England Long Term Care Foundation;past member of its Executive Committee and Secretary of the Connecticut Association of Healthcare Facilities.
Winnie Chokureva
Winnie Chokureva RN, BSN, RAC-CT, ICP, joined Strategic Care Solutions full time in October 2020 as a Senior Clinical Services Consultant and the Director Clinical Reimbursement. Prior to joining Strategic Care Solutions full time Ms. Chokureva worked with Strategic Care Solutions as an Independent Consultant on various Clinical and Clinical Reimbursement projects including Office of Inspector General (OIG) Post Payment Audits. Ms. Chokureva has been in the long-term care industry for over 26 years. Ms. Chokureva worked for Radius Management Services as a Regional Reimbursement Specialist for six years then moved on to work as a Corporate Director of Clinical Reimbursement and then a Corporate Director of Clinical Operations for Personal Healthcare for the New England Division for the past eight (8) years. Ms. Chokureva has also worked in long term care in Senior management roles for the last fifteen (15) years.
Ms. Chokureva began her career as a Licensed Practical Nurse in 1997, and then a Registered Nurse in 2003. Ms. Chokureva went on to obtain a Bachelor’s degree in Nursing from Emmanuel College, Boston MA in 2009. Ms. Chokureva’s nursing background includes Director of Nursing Services, MDS Coordinator, MMQ Coordinator, Clinical Reimbursement Coordinator, Staff Development, Infection Control Preventionist. Ms. Chokureva has worked on various projects including Office of Inspector General (OIG) Post Payment Audits, MMQ/MDS Audits, Infection Control Audits, Survey Readiness, development and implementation of Infection Control programs including development of SNF Risk Assessment and Operational Reviews. Ms. Chokureva’s expertise extends to compliance with CMS/State regulations through survey management.
Karen Koprowski, LNHA, LCSW
Karen Koprowski joined Strategic Care Solutions in 2015 as a Senior Consultant. Karen is currently the Administrator managing the Emerson Hospital Transitional Care Unit, where she is a member of the hospital leadership team. She worked at Radius Management Services for 14 years as Vice President of Operations. Prior to joining Radius, she was a Senior Administrator for 3 skilled nursing facilities, an adult day care center, and a Congregate Housing program owned by the Diocese of Worcester and the St. Vincent Healthcare System.
Her expertise to maintain compliance with CMS/state regulations through efficient survey management, deficiency free survey history and Joint Commission compliance. Karen’s business development efforts have led to occupancy improvement through the development of community Advisory Boards and legislative advocacy.
A licensed nursing home administrator and a licensed certified social worker, Karen holds a bachelor’s degree from Wheaton College, a Master’s degree in Counseling Psychology from Anna Maria College, and a Master’s degree in Public Administration with a concentration in gerontology from Clark University. She is a member of the American College of Health Care Administrators and the National Association of Social Workers.
W. Scott Plumb
W. Scott Plumb joined Strategic Care Solutions, LLC as a partner in 2014. He is currently working to collect payment from Mass Health for unpaid claims to nursing facilities for the period of 1992 – 2002 under a successful lawsuit under Janine Boudreau Healthcare Consulting Inc. Prior to joining SCS, Scott had worked in the post-acute and long term care for more than 40 years. He was at the New England Senior Care Association for 26 of those years. Mass. Senior Care Association represented more than 400 proprietary, not-for-profit and governmental skilled nursing facilities and assisted living residences caring for and employing more than 100,000 citizens of the Commonwealth.
While at Mass Senior Care Association, Mr. Plumb was the principal staff responsible for policy development in the areas of long term and post-acute care. He is an acknowledged expert on the financing of long term care and has authorized numerous articles in that area. Mr. Plumb has lectured and written extensively on a number of subjects including Medicare and Medicaid, long term care insurance, and strategic planning for post-acute and long term care providers.
Mr. Plumb holds a Bachelor’s Degree from Trinity College in Hartford Connecticut, a Master’s Degree in Social Work from Boston University, and a Master’s Degree in Business Administration from Northeastern University. He has also received numerous professional and academic awards including the Frank Manning Elder Advocacy Award from Minuteman Senior Services, the Anna V. Brown Award from the New England Association of Senior Centers and Councils on Aging, and the Northeastern University Graduate School of Business Administration award for finishing first in his MBA graduating class.
Maggie Morais
Maggie joined Strategic Care Solutions as an Accounts Receivable Specialist in 2020 after working as a contract consultant since 2018. After graduating Southeastern New England University with a B.S. in Accounting, Ms. Morais started her career working for the Diocese of Fall River in their business office. She perfected her skills and repertoire working for companies that had multiple sites in both New England and Rhode Island.
Ms. Morais has held leadership positions from Accounts Receivable Coordinator early in her career to Regional Business Office Manager and Director of Accounts Receivable where she supervised two regional account managers in 9 Skilled Nursing Facilities and 2 Assisted Living Communities. Maggie is proficient at training and supporting business office managers with AP/AR processes, monthly reconciliation reports and related collection tasks. Maggie has assisted multiple clients in receiving retroactive Medicaid approvals along with providing systems and tracking for future submissions. Over the years she has recovered millions of dollars for various SCS clients.
Tom Pogar
Tom Pogar joined Strategic Care Solutions in 2022 and brings with him four years of leadership experience as an Administrator. He graduated from Ithaca College with a concentration in Management and Marketing and a minor in Aging Studies. Tom has been instrumental in his ability to recruit talented nursing management team. His enthusiastic and energetic approach to problem resolution has brought him much success to Infection Control Surveys, Plan of Corrections, Staff restructuring for optimal efficiency, and other operational creativity.
Mr. Pogar started his career as the Director of Admissions and Marketing at Genesis Healthcare where he significantly increased census by focusing on cultivating relationships of referral sources. Mr. Pogar will be leading Strategic Care Solutions’ Interim Leadership Placement services. Tom is licensed in the states of Rhode Island and New England.
Nicole Kollman
Nicole Kollman, LPN, joined Strategic Care Solutions in 2021 with a degree from Southern Technical Institute. At Strategic Care Solutions, Nicole has been instrumental with clinical turnarounds and survey readiness training in accordance with the Rhode Island and New England Department of Health regulations. She is a licensed LPN with certification as an Infection Preventionist and IV Certified.
Prior to joining the our team, Nicole worked in several New England nursing homes as a Unit Manager and Supervisor. Nicole is licensed in Rhode Island and New England.
Tiffany Freeman
Tiffany Freeman joined Strategic Care Solutions as Office Manager in 2021. She has over 20 years of experience as a consultant, entrepreneur, and working in corporate settings. Prior to joining SCS, Tiffany worked as a consultant with many small businesses and entrepreneurs to establish policies and procedures to ensure successful operations.
She also comes with many years’ experience in the venture capital and digital marketing industries for non-profit organizations. Her tremendous insight and organizational skills brings much needed problem resolution with a creative flare to the daily back office operations.
Jacqueline Sylvia
Accomplished Chief Operating Officer of Senior Living communities and Medical Devices, expert for 25+years. Experienced Product and Clinical Development Specialist. Skilled in messaging and positioning of medical devices and disruptive technologies by market segment. Research Analytics specialist, Licensed Nursing Home Administrator (MA & NY- NHA) with experience in Turnaround Management, Performance Improvement, Medicare and Medicaid Reimbursement, Telehealth Implementation, Regulatory compliance, Quality Assurance. Successful Post-Acute Care professional with a Master of Business Administration.

Heidi Andrews
Heidi Andrews joined Strategic Care Solutions in 2021 as a member of our compliance monitoring team in the assessment of a Rhode Island facility. She has been an integral part of the success of the team by utilizing her technical expertise in areas of quality control audits, admission reviews, implementation of care plans, and staff education in areas of need.
Ms. Andrews brings great customer service and passion for others by focusing on their health and quality of life. Prior to joining our team, Ms. Andrews was involved with Lincoln Technical Institute, Herbert Health & Rehab Center along with other RI Long Term Care and Assisted Living Facilities. In her spare time, Heidi volunteers with the Glocester Emergency Management Agency doing covid vaccine clinics. Ms. Andrews holds a Practical Nursing license in the state of Rhode Island, New England, and Connecticut.
Jung Han
Jung A. Han joined Strategic Care Solutions, LLC as a partner in 2014. Ms. Han brings technical expertise in the areas of financial reporting, budgeting, operational and accounting models. Her expertise in the area of data analytics has led the path to the collection of payment from Mass Health for unpaid claims to nursing facilities for the period of 1992 – 2002 under a successful lawsuit under Janine Boudreau Healthcare Consulting Inc. Her algorithmic analysis has helped over 150 nursing facilities in New England to collect over $20 million in unpaid claims.
Prior to joining SCS, Ms. Han was a financial and budget analyst. Her technical, organizational and managerial skills have been instrumental to the successes of companies across a wide spectrum of industries like healthcare devices, biotech, investment management, opto-electronic manufacturing, food manufacturing and software-as-a-service.
Ms. Han holds a Bachelor of Science in Business Management from the University of Vermont and a Culinary Certificate from The Cambridge School of Culinary Arts.
Donna Moreno
Donna Moreno joined Strategic Care Solutions in 2021 with over 30 years of experience in nursing management and senior care. She has held the Director of Nursing positions for many skilled nursing facilities including Wingate Healthcare, Salter Healthcare, Park Avenue Nursing, and Compassus Hospice.
Ms. Moreno had achievements that brought compare ratings from 2 stars to 4 stars, maintained deficiencies below state averages, maintained JCAHO and JCC accreditations, developed a Pulmonary Program, implemented an alarm-free environment and always applied a 5-star customer service approach to patient care. Her commitment to bring value, independence and enrichment to the residents reflects in her work. Ms. Moreno is a registered RN in the state of MA, RI, and NH. She also has certifications as an Infection Preventionist.
Peter A. Middlemass
Pete Middlemass most recently served as the Division Vice President of Business Development & Provider Relations for SavaSeniorCare, overseeing the sales activities for 124 skilled nursing & assisted living facilities in the Northeast, Midwest & Southeastern States. In his role, Pete was responsible for leading the strategic sales & marketing efforts across a mixed portfolio of buildings offering multiple products and service lines. While his primary focus has been on census & network development, Pete has made significant advancements in developing and implementing innovative value-based business solutions that bridge the needs of acute and post-acute providers along the care continuum. Pete has worked closely with a variety of both large & small health systems & PHO’s to successfully partner with them to achieve their goals under various payment models and CMS/CMMI Demonstration Projects including Medicare Shared Savings Program (MSSP’s), Accountable Care Organizations (ACO’s), Bundled Payment for Care Improvement (BPCI-A) and Clinically Integrated Networks (CIN’s).
Over the past decade, Pete has worked in various Sales & Marketing leadership roles and continues to perfect his craft as our ever-changing healthcare landscape continues to evolve. A native to New England, Pete started his career in the finance world as a consultant and public speaker to CPA’ s & Accounting-software organizations. As the Author and presenter of a successful Client Sales Engagement Seminar, Pete has presented to thousands of Accounting & IT professionals throughout the United States, United Kingdom and the Middle East. Pete Holds Degrees in Business Administration from Emmanuel College, Boston & Dean College, Franklin, MA. He has served on the Board of Directors for Choice Plus PHO in Lawrence, New England and is active in several charity & fundraising events.
